Prefer paper? Mail it in.
Print the USIDA application, complete it by hand, and post it with your photos, fee and license copies. USIDA reviews it and mails your permit back to you.
Four steps, start to finish.
- Step 1
Print the blank form
Print it single-sided on plain letter paper, then fill it in with a dark pen. Write clearly — the details are copied onto the permit exactly as you write them.
- Step 2
Sign the form and both photos
Sign and date the application, and sign the back of each of your two passport-style photos so they can be matched to your application.
- Step 3
Add your fee and license copies
Include a check or money order from a U.S. bank made out to USIDA, plus a photocopy of both sides of your valid U.S. driver's license. Never send cash.
- Step 4
Post it to USIDA
Send everything in one envelope to the address below. Keep a copy of the completed form for your own records.
What goes in the envelope
- Your completed and signed IDP application form.
- Two original passport pictures, each signed on the back.
- $20 permit fee (check or money order from a U.S.A. based bank, made out to USIDA — no cash by mail).
- A photocopy of both sides of your driver's license.
- Optional: additional U.S. funds for expedited return mail.

Allow 5 business days once your envelope arrives, plus mail time each way. If you leave in 7 business days or less, visit a USIDA branch instead of mailing.
- Permits are physically printed and mailed. Digital versions are not available.
- Please allow 5 business days for processing, plus return shipping.
- If you need a permit in 7 business days or less, visit a USIDA branch near you.
- USIDA charges $20 per permit and $10 for a passport photo, which is free for USIDA members, and shipping and sales tax are added if applicable.
