I further certify that I understand a valid U.S.A. driver's license must accompany this permit when traveling, and that this permit is valid only as long as the accompanying U.S.A. license is valid, but not to exceed one year from the date the permit is issued.

- Your completed and signed IDP application form.
- Two original passport pictures, each signed on the back.
- $20 permit fee (check or money order from a U.S.A. based bank, made out to USIDA — no cash by mail).
- A photocopy of both sides of your driver's license.
- Optional: additional U.S. funds for expedited return mail.
Submission deadline: mail this form at least 13 business days before you leave (5 business days of USIDA processing plus return mail). If you leave in 7 business days or less, visit a USIDA branch instead.
